Spoilers through House of the Dragon season 3, episode 2. Otto Hightower was held in the Black Cells on Larys Strong’s orders. After Rhaenyra takes King’s Landing, she executes him. Season 2’s finale showed Otto imprisoned without identifying the cell or explaining how he got there; season 3, episode 2 supplies those answers.
What happened to Otto Hightower?
After Aegon dismisses him as Hand, Otto leaves King’s Landing. He intends to travel to Oldtown to meet Prince Daeron, while Alicent wants him to secure House Tyrell’s support at Highgarden, according to Esquire’s account of the season 2 finale.
Otto then disappears from the story and stops answering Alicent’s letters. The season 2 finale ends his absence with a brief shot of him behind bars, but does not reveal where he is or who captured him. Showrunner Ryan Condal described the scene at the time: “We find Otto in this cell somewhere, and we don’t know quite where he is or what happened to him.” That was the information available at the end of season 2, not the show’s final explanation.
Where was Otto imprisoned?
Season 3, episode 2 identifies Otto’s prison as the Black Cells and says Larys Strong ordered his capture. Larys kept him there so he would be available when Rhaenyra took King’s Landing, according to Sky’s episode explainer.

How does Otto Hightower die?
After Rhaenyra takes the capital, Daemon brings Otto before her and urges her to execute him. Rhaenyra carries out the execution herself. The show’s account is described in Sky’s recap of the episode.

The television series’ imprisonment storyline should not be treated as an event established in George R. R. Martin’s Fire & Blood. The book does not describe Otto’s capture in the interval between his removal as Hand and the Battle of the Gullet, according to The Escapist.
The accounts also differ in what they establish about Otto’s execution. Fire & Blood says Rhaenyra orders his death but, as SYFY Wire summarizes, does not name the person who carries it out. The series makes Rhaenyra the executioner. The adaptation changes other parts of Otto’s political role as well: Esquire notes that the book has Otto negotiate with the Triarchy, while the show gives that task to Tyland Lannister.
