Disney+ identifies Isaac’s episode as “Isaac’s Journey,” Season 9, Episode 8 of My 600-lb Life (Disney+ episode listing). According to a February 2021 recap by Distractify, Isaac was 23 and from Hutchins, Texas, and began the episode at 661 pounds. The recap reports that he lost 122 pounds over the year covered by the episode (Distractify, February 17, 2021).

Those figures come from the recap, not an independently published TLC statistical record. The same article says Dr. Now conditionally approved Isaac for surgery if he continued making progress and lost another 30 pounds. That was approval contingent on further progress, not confirmation that surgery took place.
Did Isaac Martinez get weight-loss surgery?
The available sources do not establish whether Isaac eventually underwent weight-loss surgery. Distractify’s 2021 account describes conditional approval, while TLC’s later follow-up listing does not say that an operation occurred. Approval to proceed and completion of surgery are different milestones; the sources support the former, not the latter.

What did the 2025 follow-up reveal?
TLC lists Isaac in a My 600-lb Life: Where Are They Now? episode titled “Isaac & Irene.” Its official synopsis says, “Putting family first has kept Isaac from getting the help he needs” (TLC episode listing). TV Guide dates the episode to April 9, 2025, and describes Isaac as having come close to surgery multiple times while putting family first (TV Guide, Season 10 episode guide).
The synopsis indicates that family responsibilities affected his pursuit of care, but it does not provide a verified surgery result, current weight, or detailed account of his circumstances after the episode.

Where is Isaac now?
The sources reviewed identify a 2025 follow-up but do not verify Isaac’s present-day circumstances or provide an update after that episode.
