What happens at the end of the Season 12 premiere?
“Ten Ninety-Nine” introduces Emily Martel, played by Victoria Cartagena, as a new detective in the Intelligence Unit. In the episode’s closing attack, shots are fired while Martel and Ruzek are in a car. Martel is shot in the head in front of him. The next episode follows Ruzek and the unit as they deal with her death.

The twist is abrupt, but its importance is not just the shock of the moment. Showrunner Gwen Sigan connected Martel’s death to Voight’s near-death experience in the preceding season and to Season 12’s themes of mortality, identity, and transformation. She said the writers wanted to underscore that death can arrive without warning, leaving no time to prepare, say goodbye, or tie up loose ends.
Patrick John Flueger said Ruzek had known Martel for more than a decade and described this as the first time his character had watched a friend die so directly on screen. That backstory describes Ruzek’s relationship with Martel; it does not mean viewers had been following her for ten years.

How does Martel’s death echo the series pilot?
The comparison is about the shape of the storytelling: a shocking death at an episode’s end that changes the emotional direction of the story. A September 2024 ScreenRant headline described Martel’s fate as a parallel to the pilot’s twist. The available account supports that broad comparison, not a claim that the two scenes have identical victims, motives, or circumstances.
That distinction matters. “Repeats” can suggest the series is replaying the same plot, but the substantiated point is that Season 12 returns to a high-impact end-of-episode death as a narrative turn. Martel’s death launches Ruzek’s grief and feeds into the season’s mortality themes; it is not evidence of a scene-for-scene remake.

What actually happens in the Season 12 finale?
The finale is “Vows,” a separate episode with two central threads: Voight’s conflict with Deputy Chief Charlie Reid and the wedding of Burgess and Ruzek. Voight arranges for Reid to be killed by the son of a drug dealer Reid had killed. Burgess and Ruzek marry, but the episode does not show them exchanging vows; they are married by the start of Season 13.
Sigan described Reid as a “funhouse mirror” to Voight, a character designed to test whether Voight had changed. She also said the finale pushes Voight toward his darkest place. The wedding was intended to contrast its happiness and promise with the emotional cost of Voight’s actions and his ending with Assistant State’s Attorney Nina Chapman. Those are the showrunner’s explanations of the creative choices.

TV Guide’s post-finale coverage likewise focused on the question of how alike Voight and Reid are, and on the personal cost of Voight choosing his team over a possible relationship with Chapman. That finale setup is distinct from the premiere’s sudden loss of Martel.
