While traveling through Montana to find Spencer, Alex is caught in a blizzard. Her traveling companions freeze to death, leaving her to burn what she can to survive. The available episode account describes them as companions but does not identify them by name or give a precise number.
Alex eventually reaches Spencer, and the two reunite after months apart. But the reunion cannot undo the damage of her journey. Alex gives birth prematurely to their son, John, and dies after her injuries from the cold.
How the Season 1 shipwreck set an early pattern

Why Spencer and Alex were separated after the duel
In the Season 1 finale, Spencer and Alex are aboard a ship bound for London when Alex’s former fiancé, Arthur, confronts Spencer. Their duel ends with Arthur dead. Spencer is put off the ship, while Alex remains aboard and says she will meet Spencer in Bozeman.

Actor Brandon Sklenar described Spencer’s resolve this way: “He’s the kind of guy, when he makes his mind up, that’s it.” That is Sklenar’s interpretation of the character, not dialogue from the series. TVLine’s finale interview recounts the separation and includes Sklenar’s comments.
What happens when Alex finally reaches Spencer
The Season 2 finale, Episode 7, is titled “A Dream and a Memory,” according to Paramount’s official 1923 press index. In the episode, Alex and Spencer do find each other after their separate journeys, but Alex is gravely ill following the blizzard and gives birth prematurely before she dies.

TV Insider’s account of the finale says Alex’s limbs were necrotic, doctors recommended surgery and abortion, and she refused, choosing to keep the baby. The account says John survives. These are details of the episode as reported by the outlet, not real-world medical guidance. TV Insider also reports that Spencer lives for decades and later dies beside Alex’s grave. The connection is a pattern across Spencer and Alex’s backstory, not a separate origin event. They survive a shipwreck together, are split apart after Arthur’s death, and later face another dangerous journey that ends with Alex’s death soon after their reunion. Those events help explain the losses surrounding Spencer’s story in 1923; the phrase “tragic origin” is an editorial description of that history, not wording attributed to the series or its sources.
