How did Richard Matt and David Sweat get out?
Matt and Sweat escaped through a route cut from their cells into the prison’s underground spaces and out beyond the facility. The Inspector General’s findings show that the tunnel was only one part of the story: civilian employee Joyce Mitchell provided assistance, correction officer Gene Palmer acted recklessly, and routine controls failed to detect or stop the plan.

Employee assistance and reckless conduct
Mitchell, who worked in the prison tailor shops, assisted the inmates. Palmer’s conduct was separate: the Inspector General found that his reckless actions also helped them. It is therefore inaccurate to reduce the escape to a single guard secretly helping the men. The state’s investigation considered the actions of employees alongside failures across the facility.
In announcing the findings, Inspector General Catherine Leahy Scott said: “The extent of complacency and failure to adhere to the most basic security standards uncovered by my investigation was egregious and inexcusable,” the Office of the Inspector General’s release reported.

Missed counts, searches and inspections
The investigation identified failures in employee entry screening, night counts, cell searches, supervision, tunnel and catwalk inspections, and management oversight. These lapses allowed the men to work on their escape over time and left opportunities to discover it unused.
For approximately 85 nights while Sweat was working in the tunnels, more than 400 bed checks should have taken place. The report says a properly conducted check would have detected his absence. In another example, a recorded search of Matt’s cell failed to find a hole in the rear wall measuring 18½ by 14½ inches. These were not obscure technical failures: they were missed opportunities in ordinary security routines, as detailed in the Inspector General’s investigation.

What happened to Matt and Sweat?
The escapees remained at large for three weeks. According to the New York State Police’s 2015 annual report, Matt was shot and killed on June 26 after he refused commands and pointed a shotgun at U.S. Border Patrol’s BORTAC team. Sweat was captured alive near the Canadian border on June 28. These details are from the State Police’s account of the manhunt, not a scene-by-scene reconstruction of the television series.
How accurate is Escape at Dannemora?
The series is a scripted dramatization based on the escape, not a documentary. The New York State Inspector General’s 2025 tenth-anniversary notice says its investigation provided a roadmap for the miniseries.
