Sylvester Stallone said he got a tattoo of his wife to cover the visible appearance of chest injuries, and that the design grew into a skull and then a mural. A 2008 TIME profile described the sleeve’s imagery as a portrait of his wife, three roses and a tiger, and reported meanings for the roses and tiger. Those accounts explain what was reported at the time; they do not provide a complete inventory of his tattoos today.
Why did Stallone get a tattoo of his wife?
In a 2013 interview with Shortlist, Stallone said chest injuries had left marks that became increasingly noticeable. He explained that he covered them with a tattoo of his wife, and that the artwork expanded over time.
“They’re real – unfortunately. I have all kinds of chest injuries; I tore my vein during Rocky II and had 60 stitches. If you’ve seen Rocky Balboa, you’ll know how bad they look. It got worse and worse, and people were saying, ‘Urgh, has he got varicose veins?’ So I thought, f*ck it, and covered them with a tattoo of my wife. Then the tattoo started to grow and it went to a skull and before I knew it, I was a mural.”

This is Stallone’s own explanation for the first tattoo and its development. His account of the injuries is an interview statement, not an independently verified medical history.
What does the sleeve tattoo depict?
A 2008 TIME profile described a large color portrait of Stallone’s wife on his right sleeve, surrounded by three roses and watched over by a tiger. The profile connected the roses to the middle name shared by each of his three daughters, and the tiger to Rocky’s fondness for tiger eyes. That is TIME’s account of the imagery and symbolism; it should not be taken as Stallone’s definitive explanation of every design element.

Are Sylvester Stallone’s tattoos real?
Yes. In the 2013 Shortlist interview, Stallone explicitly said they were real and discussed getting the tattoo to cover the appearance of his chest injuries.
