Yes: the season finale reveals a physical power linked to Billy Butcher’s tumor. The clue comes earlier, in Episode 6, when the show makes clear that Joe Kessler is a hallucination—not an active partner in Butcher’s present-day story.
What is Butcher’s power?
In the Season 4 finale, Butcher’s body produces black, tumorous tentacles. A draft of the finale script describes them emerging from his chest, providing a literal payoff to the season’s hints about his illness and visions. That scene establishes a disturbing physical manifestation; it does not, by itself, explain the power’s full limits, biology, or future uses.

The two reveals are related but distinct: Episode 6 exposes the tumor’s role in Butcher’s visions, while the finale shows the tentacles. The first is a psychological clue; the second is the on-screen power reveal.
When is Butcher’s power revealed?
Episode 6: Kessler is a hallucination
Butcher’s apparent partnership with Joe Kessler initially seems to be part of the season’s present-day action. Episode 6 reveals that Kessler is a hallucinatory figure associated with Butcher’s brain tumor, rather than a real partner participating in those events. TVLine’s episode recap describes the twist.
#1 Best Overall
Showrunner Eric Kripke connected the visions to both the illness and Butcher’s darker impulses. In a TV Insider interview about the episode, he said: “I just think it’s cancer and the dark side of Butcher that have all metastasized into the same thing.”
The finale: the tumor manifests physically
The finale moves beyond the hallucination clue and depicts the tentacles. The available description comes from a draft of the finale script, which calls them “black, tumorous tentacles” emerging from Butcher’s chest. That supports the scene’s visual description, but it is not an official episode transcript or a complete account of how the power works.

Is Joe Kessler real in Season 4?
No—not as the active partner Butcher appears to be dealing with. Episode 6 identifies Kessler as a hallucination tied to Butcher’s tumor. The reveal reframes their conversations and gives the tumor a clear role in the season’s story; it does not mean that every aspect of Butcher’s illness or new ability is medically explained.
