Yes. Madelyn Stillwell dies in The Boys Season 1 finale, “You Found Me,” when Homelander kills her. Her later appearance in the Season 5 finale is described as a hallucination—not a resurrection.
How Madelyn Stillwell dies
Prime Video lists “You Found Me” as the Season 1 finale, released July 26, 2019. Prime Video’s episode listing identifies its place in the season.
In the finale, Billy Butcher uses Stillwell as bait to get at Homelander. Homelander has learned that she concealed the existence of his son, Ryan. He confronts her and kills her with his heat vision. Butcher’s explosive trap does not kill Homelander. TVLine’s recap describes the confrontation and its aftermath.
Why she appears again in Season 5
A recap of the Season 5 finale describes Stillwell appearing to Homelander as a hallucinated, angelic figure. The scene depicts an image in Homelander’s experience; it does not establish that Stillwell survived her Season 1 death or returned to life. GamesRadar+’s May 20, 2026 recap characterizes the appearance as a hallucination.

How the television twist differs from the comic
The Boys television series adapts Garth Ennis and Darick Robertson’s comic-book series. Executive producer Eric Kripke discussed how the finale twist developed during the writing process and how it differed from the comic in a 2019 interview with SYFY WIRE. That adaptation context helps explain why comparing the show’s events with the comic can produce different answers; Stillwell’s status in the television story is settled by what happens on screen in “You Found Me.”
