The precise technical sequence that made the firearm dangerous is not established in the reporting cited here. It is therefore more accurate to say that Lee was shot during filming and that investigators later attributed the death to negligence than to claim a specific cartridge, handling error, or inspection failure.
What did investigators conclude, and was anyone charged?
District Attorney Jerry Spivey concluded that crew negligence caused Lee’s death, but declined to bring criminal charges. The decision reflects an important distinction: a finding of negligence does not automatically meet the legal standard required for criminal prosecution. The Los Angeles Times reported that the prosecutor found no evidence of the “willful and wanton” negligence required to prosecute.

The Washington Post reported Spivey’s conclusion that the death “was the result of ignoring basic and well-recognized safety guidelines.” That is a reported statement by the prosecutor, not a technical account of exactly how the shooting occurred.
Was Brandon Lee’s death part of a plot?
No evidence of intentional harm or a plot was found in the reported investigation. Accounts describe the death as an accident caused by negligence, not a planned or staged event. The Washington Post’s account of the prosecutor’s findings and the Los Angeles Times’ report on the charging decision support that distinction.

What happened to The Crow after the shooting?
Production resumed, and the film was completed and released in 1994. It was dedicated to Lee and his fiancée, and was based on James O’Barr’s comic. The Guardian’s retrospective account covers the film’s completion and dedication; Britannica’s overview identifies the comic as its source.
