What does “highest-grossing” mean when adjusted for inflation?
There is no single inflation-adjusted ranking that answers every version of the question. A list may cover worldwide or North American theatrical revenue, estimate ticket admissions and multiply by a modern ticket price, or adjust recorded historical grosses to a chosen dollar year. It may also count theatrical rereleases differently.

Those choices matter especially for films released decades ago. Ticket prices have changed, archival box-office records are uneven, and a long-running title can accumulate revenue across multiple rereleases. The result is best read as an estimate with a stated scope and method—not as a precise count of people who watched a film.
The worldwide record: Gone with the Wind
Guinness World Records names the 1939 film as the global inflation-adjusted record-holder. Its current record page gives two estimates with different descriptions. The first is $3.44 billion, calculated by estimating total tickets sold and multiplying by the current average U.S. ticket price. Guinness separately describes an estimated inflation-adjusted gross of $4.5 billion that includes worldwide box-office numbers and theatrical rereleases. Guinness states its ticket-based calculation this way: “The total was worked out by estimating the total number of tickets sold for a film and multiplying it by the average U.S. ticket price for the current year.” That produces an estimated gross, not a verified ledger of every ticket sold around the world. Nor does an admission count equal a count of unique viewers: one person can buy tickets for multiple visits.
North American adjusted grosses: Box Office Mojo’s 2020-dollar chart
Box Office Mojo’s adjusted-gross chart is a domestic comparison, covering the United States and Canada rather than worldwide grosses. With the chart set to 2020 dollars, its top four are:

Why the target dollar year changes the totals
An inflation-adjusted amount needs a comparison year. Box Office Mojo’s 2000-dollar setting keeps the same top four in the retrieved chart, but gives lower adjusted totals than its 2020-dollar setting:

Rereleases, record gaps and revenue categories
Rereleases complicate a film’s lifetime total
Gone with the Wind has returned to theaters many times. The Numbers records numerous domestic releases from 1939 through 2026, so totals can differ depending on whether they include later theatrical runs and how those runs are recorded. The Numbers lists a nominal worldwide theatrical gross of $390,516,513, split into $198,680,470 domestic and $191,836,043 international. That is a separate, unadjusted theatrical figure and a useful cross-check, not a definitive reconstruction of every admission worldwide. Historical records are not equally complete
Guinness cautions that early box-office records are less comprehensive and ticket prices fluctuate. Those limitations make inflation-adjusted historical rankings more open to interpretation than a simple comparison of recent reported grosses. Theatrical grosses are not home-video sales
The Numbers separately estimates $80,634,815 in domestic DVD and Blu-ray sales for Gone with the Wind. That is home-video revenue, not theatrical box office, and should not be added to the film’s theatrical gross when comparing box-office records. Inflation-adjusted and unadjusted records are different claims
The worldwide film with the largest unadjusted gross is not necessarily the inflation-adjusted record-holder. Guinness’s account of Avengers: Endgame overtaking Avatar concerns the unadjusted global-gross chart; Guinness identifies Gone with the Wind for the inflation-adjusted record.
