It is not a reliable account of human physiology: an ENT specialist quoted in a 2017 report said that, although blood pressure can rise during arousal, there is no established connection between arousal and nosebleeds.
What does an anime nosebleed mean?
The convention makes a character’s reaction instantly legible: a nosebleed stands in for arousal or overwhelming attraction. It is an exaggerated comedic sign, not a medical diagnosis or a realistic depiction of what normally happens when someone is attracted to another person. Robin E. Brenner describes the nosebleed as a manga and anime convention in a chapter excerpt from Understanding Manga and Anime (2007): Google Books.
Where did the trope come from?
The exact first use has not been established. A 2017 ComicBook.com report says the innuendo is believed to have been used by manga artist Yasuji Tanioka in the 1970s. That makes Tanioka a reported attribution, not a confirmed inventor. An academic chapter published in 2020 likewise describes the convention’s history as uncertain and notes that multiple explanations have been proposed: “Meaning-(re)making in a world of untranslated signs”.
The most defensible explanation is about the trope’s function: manga and anime use visual exaggeration to convey a character’s reaction quickly, and the nosebleed became a recognizable comedic code. Popular claims that it began specifically to evade censorship or depict ejaculation should not be treated as documented history; the cited sources do not establish that origin story.

Can arousal cause a nosebleed in real life?
A sudden nosebleed triggered by seeing an attractive person is a fictional exaggeration, not an established physiological response. In the 2017 report, ENT specialist Dr. Koichiro Kanaya is quoted saying: “It’s true that blood pressure rises when we’re sexually aroused, and it’s been well documented, but there is no connection between arousal of this kind and nose-bleeds.” The quotation is reproduced through that secondary report; its original interview was not independently reviewed.
Real nosebleeds, also called epistaxis, usually involve the delicate lining inside the nose, where small blood vessels lie close to the surface. The NHS guidance on nosebleeds lists causes including picking, forceful nose-blowing, dryness, injury, some conditions affecting blood vessels or clotting, and certain medicines. Cleveland Clinic similarly explains that dry air can irritate the nasal lining: Nosebleeds (Epistaxis): Causes, Treatment & Prevention.

Cleveland Clinic estimates that about six in ten people experience at least one nosebleed during their lifetime. That is a general prevalence estimate, not evidence of a link between nosebleeds and sexual arousal.

Most nosebleeds have ordinary physical explanations, but recurrent or significant bleeding is a medical issue rather than a trope. The American Academy of Otolaryngology–Head and Neck Surgery Foundation published a clinical practice guideline on nosebleed evaluation and management in 2020: Clinical Practice Guideline: Nosebleed (Epistaxis) Executive Summary. Individual assessment and treatment depend on the circumstances; the anime convention offers no guidance for handling an actual bleed.
