Spoilers for all three seasons, especially Season 3, Episode 8, “This Is a Story.” The finale confirms that the narrator is an older Gus telling the history of the world to a new generation of hybrid children. Gus burns the tree in the cave, and the show presents its destruction as the end of the Sick. But Jepperd’s fate is deliberately left unresolved: the final Yellowstone reunion may be real, or it may be the hopeful ending Gus gives Big Man in a story as Jepperd is dying.
Who is narrating Sweet Tooth?
The narrator is an older Gus, telling his grandchildren how the world before them changed. The final scenes show the hybrid community he describes: the future Gus imagines for the children becomes the world they inhabit.

Showrunner Jim Mickle said the older-Gus framing connects the adaptation to the comic’s ending, while giving the series a storybook quality. Netflix describes the show as based on Jeff Lemire’s comic; its third season is the final eight-episode chapter of a 24-episode series. What happens to Gus at the end?
Gus uses a flare to burn the tree in the cave, the tree associated with the Sick. In the show’s fictional resolution, the Sick ends as the tree dies; the cave collapses, and the narration describes humans as the last humans and hybrids as inheritors of the Earth. This is the story’s supernatural, speculative outcome—not a claim about real disease or biology.

Dr. Singh saves Gus from falling debris by throwing himself between the boy and the collapse. Singh is crushed and dies. His final act is a sacrifice that protects the child he had pursued.
Does Big Man die? Did Jepperd make it back?
The series does not give a definitive on-screen answer. Zhang stabs Jepperd, leaving him gravely wounded. As he and Gus part, Jepperd asks whether Big Man made it back. Gus does not answer directly. The show then cuts to the Yellowstone reunion, but that scene is open to interpretation rather than a firm confirmation of survival.

The tragic interpretation draws support from Jepperd’s grave wound, his question, and older Gus’s description of Jepperd as someone who “lives on in our stories.” The literal reading takes the Yellowstone image at face value. Mickle and actor Nonso Anozie have said the choice belongs to viewers; Mickle put it simply: “I leave it up to you.” Actor Christian Convery offered his own view: “I personally don’t think he does.” Those are interpretations, not a canon answer.
Why does Birdie die?
Birdie is killed before the resolution. Mickle told TVLine that versions in which she survived felt too neat for the story. Her death forces Gus—who tends to see the best in people—to confront human cruelty and pushes his innocence toward a breaking point. It also avoids an uncomplicated family reunion in which everyone simply lives happily ever after. Mickle’s TVLine interview explains that choice and his approach to the ambiguous ending.

What is the ending saying about the future?
The finale gives the hybrids a future community after Gus destroys the tree and the Sick ends in the show’s account. That outcome is not presented as a return to the old human world: Gus’s narration says hybrids will inherit the Earth. Yet Jepperd’s uncertain fate complicates the apparent optimism. Whether he survives or only lives on in Gus’s telling, the ending makes storytelling part of how Gus carries loss into the future.
Mickle described the emotional hope behind the series this way: “I think we can allow ourselves to feel empathy towards one another and I hope that the show greases the wheels for that a little bit.” Older Gus’s final line to the children captures the forward-looking frame: “All stories end, but ours has just begun.”
