It is a candid image, not one of the formal coronation portraits released in May 2023.
What the birthday photo shows
The photograph captures Catherine smiling toward King Charles, who is wearing the Imperial State Crown. Prince Edward and Sophie, Duchess of Edinburgh, are also in the scene. ITV News reports that it was taken inside Buckingham Palace after the coronation ceremony; the available reporting does not establish whether it came before or after Charles and Camilla’s first appearance on the balcony. ITV News and Town & Country describe the scene and its timing.

What the King’s birthday message said
Harper’s Bazaar reproduced the greeting from the official Royal Family social post: “Wishing the Princess of Wales a very happy birthday today! 🎈🎊” The message accompanied the coronation-day photograph. Harper’s Bazaar reported the post on January 9, 2024.

How this image differs from the official coronation portraits
The birthday photo is a behind-the-scenes moment credited to Chris Jackson and shared in January 2024. It is separate from the Royal Family’s formal coronation portraits, announced on May 8, 2023. Those portraits were taken by Hugo Burnand in the State Rooms after the service. The distinction is useful when searching for the “Kate Middleton birthday photo” or the official coronation images: they are different photographs, made for different purposes and released at different times. The Royal Family’s official portrait announcement identifies the formal set.

ITV describes Catherine wearing a deep-blue Royal Victorian Order mantle edged in scarlet and silver bullion, with a crystal-and-silver embroidered leaf headpiece. Harper’s Bazaar identifies the headpiece as a Jess Collett x Alexander McQueen collaboration and reports that her dress was by Alexander McQueen. These details identify what she wore in the image; they do not make the photograph itself a fashion portrait.
