Spoilers for the Jujutsu Kaisen manga: Yes. Yuji Itadori and Choso are brothers in the story’s supernatural sense. Kenjaku’s role in their origins connects them, and Choso comes to recognize Yuji as his younger brother. That is a meaningful family bond, but it is not a straightforward claim that they share an ordinary mother or a conventional sibling genealogy.
How are Yuji and Choso related?
The connection runs through Kenjaku, whose actions are bound up in the origins of both characters. Choso is one of the Death Painting Wombs, created through Kenjaku’s involvement; Yuji’s conception is also connected to Kenjaku. The story therefore gives them a genuine supernatural family link, even though it does not fit the usual account of siblings sharing the same two parents.

The distinction matters: calling Choso Yuji’s “big brother” captures how Choso sees and treats him, but it does not explain the unusual lineage behind that bond. The manga’s revelations are better understood as a supernatural connection shaped by Kenjaku than as an ordinary family tree.
Why does Choso recognize Yuji as his brother?
Choso’s sense of kinship becomes part of the plot rather than a casual nickname. In material around chapter 143, Yuji dreams of his mother, and the resemblance between her forehead scars and Kenjaku’s points toward the connection. Later family-history disclosures clarify why Choso’s bond with Yuji belongs alongside his relationship to the other Death Painting Wombs.

Chapter 203 also emphasizes Choso’s commitment to protecting his brothers and his complicated relationship with Kenjaku. Together, these developments frame Choso’s recognition of Yuji as part of the story’s family history as well as an emotional attachment. The official VIZ chapter catalog lists both chapter 143 and chapter 203; the listing confirms the chapters, while the manga itself is the source for their plot details.
Are they biological brothers in the usual sense?
Not in the simple, everyday sense of two siblings with a conventional shared-parent genealogy. The manga ties their origins together through Kenjaku and supernatural creation, but that is not the same as establishing that Yuji and Choso have the same ordinary biological mother. It is more precise to say that they are brothers through the story’s supernatural lineage, and that Choso regards Yuji as his younger brother.

That wording preserves both parts of the answer: their connection is real within the story, and its mechanism is unusual. It avoids turning an emotionally accurate description of their relationship into an unsupported claim about a conventional family tree.
