Iron Lung is a 2026 science-fiction horror film written and directed by Mark Fischbach, better known as Markiplier. It adapts David Szymanski’s 2022 submarine horror game: a convict is sent in a cramped submersible to investigate an ocean of blood on a desolate moon. The film is fiction—not a documentary about the medical device called an iron lung.

How does the film adapt the game?
The game and film share the premise and the claustrophobic submarine setting, but they use different ways to build tension. Steam describes the game as a short, single-player horror experience in which players navigate using an incomplete map, proximity sensors, sound, and a grainy external camera. Its intended playthrough is under an hour, according to the Steam listing.
For the film, Fischbach said the creative team’s guiding rule was to stay inside the submarine: “our first rule was staying in the submarine.” In a February 23, 2026 interview with YouTube’s Official Blog, he described the deliberate pace this way: “The point is the monotony, the loneliness, the boredom.” The restricted viewpoint is part of the adaptation’s method, not simply a visual similarity to the game.
The film does not need to reproduce the game’s controls to preserve its central unease. A player experiences limited information through navigation and tools; a film audience follows a character trapped with the consequences of what he can—and cannot—see. Fischbach also explained the balance between mystery and clarity: “There’s a difference between "not telling you anything" and "can’t tell you anything."”
When did Iron Lung come out?
Regal announced a theatrical opening for January 30, 2026, with pre-opening shows listed for January 29. That announcement is historical, not confirmation of current screenings; consult local listings for any present-day showtimes. The original game was released on Steam on March 9, 2022.
