First Incision is an educational podcast by surgeon Amanda Nikolic for doctors preparing for the general surgery Fellowship of the Royal Australasian College of Surgeons (FRACS) examination. In 2024, Fiona Stanley Fremantle Hospitals Group reported that the show had been streamed more than 250,000 times. That is a milestone reported in 2024, not a verified current total.
What is the First Incision podcast?
First Incision is a general surgery exam revision podcast created by Amanda Nikolic. It is aimed at surgical trainees preparing for the FRACS general surgery fellowship examination, with episodes organized around areas of the general surgery curriculum. The Apple Podcasts listing describes the show as exam preparation; its catalogue details can change over time.
Who is Dr Amanda Nikolic?
Nikolic is a surgeon whose work includes colorectal surgery. Fiona Stanley Fremantle Hospitals Group described her as a colorectal fellow at Fiona Stanley Hospital in its October 2024 announcement. She began the podcast while studying for her own fellowship exams.
Why did she start the show?
Nikolic has said she searched for a podcast that could help with general surgery fellowship study but found few options with an Australian flavour. During Melbourne’s 2020 lockdown, she bought a microphone and began recording. The idea was to talk through curriculum topics and make that learning useful to other trainees as well as herself. She told PerthNow that the podcast was “a way for me to talk out loud about different curriculum topics and for others to learn along with me.”

What does First Incision cover?
The show covers multiple areas of general surgery rather than one specialty. Its listed curriculum topics include:
- Breast, upper gastrointestinal and colorectal surgery

Apple Podcasts listed 120 episodes and years active from 2020 to 2025 when accessed on 8 October 2026. Episode counts are platform catalogue information, not a fixed measure: PerthNow reported more than 100 episodes on 20 September 2024, while AusDoc described nearly 120 on 15 October 2024.

What does “more than 250,000 times” mean?
Fiona Stanley Fremantle Hospitals Group reported the more-than-250,000-stream milestone in October 2024; PerthNow reported the same milestone in September 2024. Neither figure establishes how many streams the podcast has accumulated since then, so it should be understood as a historical 2024 milestone rather than a current count. Nikolic told PerthNow she had not expected the show to become so popular and was glad listeners found it valuable.

First Incision can serve as an educational revision aid for trainees reviewing general surgery topics. Its published disclaimer says it is not a substitute for candidates’ own preparation and that listeners should rely on their independent analysis. Listening should therefore complement—not replace—formal training, broader exam study, or clinical judgment.
