In a 2020 interview, Keaton said she hoped telling their story would help open a dialogue about mental health and encourage people to care for loved ones earlier.
What Keaton’s memoir tells about her brother
Brother & Sister: A Memoir reconstructs the siblings’ lives from childhood onward. In a 2020 profile for The Washington Post, Ellen McCarthy reported that Keaton’s account brings together her recollections with journal entries by Randy and their mother. The book is a family story told through those different perspectives, rather than a clinical account of Randy’s health.
Keaton acknowledged that she had prioritized her career and fame while Randy was struggling. She wished she had offered him more love and attention sooner. As McCarthy reported her saying, “There are so many people who live through the pain of having a family member who doesn’t quite fit in.”
How the siblings reconnected
McCarthy’s February 17, 2020, report describes the siblings as having begun to spend time together regularly in the years before publication. At that time, Keaton visited Randy on Sundays. She recalled those visits as meaningful: “All I remember about that was how special those times were with him. It was like opening something up to me. Giving me such a gift.”

The same report said Randy had Parkinson’s disease and dementia and was living in assisted care when the article was published. Those are details reported in 2020; that coverage does not establish his present-day health or living situation.
What is—and is not—established about Randy’s diagnosis
A People interview syndicated by Yahoo Entertainment and updated January 30, 2020, described a range of challenges and diagnoses associated with Randy, while reporting that no definitive diagnosis had been reached. It would therefore be inaccurate to present one specific mental-health label as settled. Keaton’s story is her account of her brother and their family, not general clinical guidance.

The connection to Annie Hall
The People interview reporting says Randy inspired Duane, the troubled brother played by Christopher Walken in Annie Hall. Keaton characterized the film portrayal as a cartoon version of her brother; the reporting emphasizes that Randy’s actual experience was more complex than the character.

In McCarthy’s 2020 Washington Post profile, Keaton said she wanted the memoir to open a dialogue about mental health. She also hoped that sharing her regret would serve as a cautionary tale: pay attention to loved ones sooner, rather than waiting until years have passed. The account does not claim that every family’s circumstances are the same; it documents what Keaton came to understand about her relationship with Randy.
