It depends on the setting. “Elf” is not one universal fictional species: some stories and games give elves magical ways to change their bodies or appearance, while others do not. In Dungeons & Dragons, current elf rules describe an ancient form-changing ability that elves lost; older lore also includes a specific blessed-elf precedent for changing sex. Neither answers every question about gender identity, which is distinct from anatomy or magical transformation.
What does “change gender” mean here?
The phrase can refer to different things: a character’s gender identity, how other people recognize them, a change in sex or anatomy, or a magical change in physical form. Fiction may treat these separately. A character does not need a magical transformation for their gender identity to be real, and a spell or shape-changing trait does not by itself establish anything about identity.
For a useful answer, identify the fictional setting and edition, then ask what the relevant source actually says changes. A form-changing power, an appearance change, and a sex-change ability are not interchangeable claims.
What do Dungeons & Dragons sources say?
Current D&D Beyond elf species page
The current D&D Beyond elf species page says that the first elves, created by Corellon, could change their forms at will. It also says elves lost that ability after Corellon cursed them, and that it did not return when most elves later earned forgiveness. The page’s listed traits include lineage spells, Fey Ancestry, Keen Senses, and Trance; it does not list a general sex-change trait. This describes that page, not every optional rule or setting book published for D&D.

Legacy blessed-elf lore
Older D&D material provides a more specific precedent: “blessed” elves associated with Corellon and changing sex. Brandon Andrew Berge discusses this lore in the 2023 scholarly article “The Table and the Tomb: Positioning Trans Power and Play Amid Fantasy Realism in Dungeons & Dragons”, citing Mordenkainen’s Tome of Foes. That article is scholarly interpretation of the material, rather than publisher-authored rules text. The precedent belongs to that lore; it is not a blanket rule for every elf in every D&D world.
Eladrin seasons and appearance
A separate example concerns Eladrin, whose season association can change after a long rest. In an official D&D Beyond interview about Mordenkainen’s Tome of Foes, rules designer Jeremy Crawford describes possible visible effects: “If you change to autumn, for instance, your hair might take on autumnal colors. Your skin color might change. If you change to winter, you might have a blueish complexion. You might even have some icicles in your hair because, again, you are a creature of magic and you are like the first elves back before they were cast out and lost most of their divine nature.” The example supports seasonal and appearance changes; it does not say that changing seasons changes sex.

What about Tolkien’s elves?
Tolkien’s Eldar should be treated as a separate fictional tradition, not folded into D&D rules. Tolkien Gateway’s secondary summary of “Laws and Customs among the Eldar” describes childbearing and differing typical inclinations, while saying that male and female Eldar are considered equal in matters not concerning childbearing. That summary discusses sex, reproduction, and roles; it does not establish that Tolkien’s elves can change sex. The equality wording is the summary’s wording, not a verified direct quotation from Tolkien.

Check the source’s scope before applying a claim broadly. A useful distinction is whether it describes an ordinary species trait, an optional character choice, a divine blessing, or a setting-specific event. Then check whether the stated change concerns identity, social recognition, sex or anatomy, or only outward appearance. A rule for one edition or group of elves should not automatically be applied to another.
