Spoilers for the full film. Cillian Murphy returns as Jim in the final scene of 28 Years Later: The Bone Temple: alive, living with his teenage daughter Sam, and ready to help Spike and Kelly as they flee infected. The film cuts away before showing the rescue, so Jim’s intervention is set up rather than completed.
What happens at the Bone Temple?
Jimmy Crystal and his Jimmies arrive at Kelson’s Bone Temple. Jimmy Ink has come to believe Kelson is Old Nick, the devil figure Crystal says is his father. Kelson plays into that belief with fire, music, and dance, performing for the group.

When Kelson recognizes Spike among the masked followers, he refuses to continue following Crystal’s orders. The group turns against its leader. Spike survives, and Crystal is crucified on the inverted-cross prop. Before dying, Crystal fatally stabs Kelson. This sequence is detailed in Radio Times’ spoiler recap.
Does Kelson cure Samson?
Kelson’s treatment produces a striking result for Samson: the infected man speaks clearly and recovers memories. Samson later returns to thank Kelson before Kelson dies. The film presents this as a breakthrough in Kelson’s work, but the scenes establish Samson’s immediate response, not a lasting or universal cure.

How does Cillian Murphy return as Jim?
After the confrontation, Spike and Jimmy Ink escape together. Ink tells Spike her real name is Kelly. As the two flee infected, the film cuts to a familiar cottage, where Jim is alive and raising his teenage daughter, Sam. They see Spike and Kelly in danger, and Jim agrees to help.
The ending confirms Jim survived and gives him a direct reason to cross paths with Spike. It does not show the rescue or explain what Jim will do afterward. In an interview with GamesRadar+, director Nia DaCosta described directing Murphy’s return: “I loved it. I was so excited to do it. I was like, ‘Yes, I get to do this!’ Like, you know, bring this guy back.” Her comment describes her excitement about the casting and scene; it does not spell out the ending’s meaning.

What does the ending set up?
The Bone Temple continues directly after 28 Years Later, following Spike’s encounter with the Jimmies and Kelson’s work on Samson, according to Sony Pictures’ official film page and its official FAQ. The closing scene hands the story toward Jim’s possible involvement in Spike and Kelly’s escape, but leaves the outcome unresolved.
Sony’s official synopsis describes the film’s conflict this way: “In the world of The Bone Temple, the infected are no longer the only threat to survival – the inhumanity of the survivors can be stranger and more terrifying.” That framing fits the climax: Kelson’s theatrical deception briefly disrupts Crystal’s hold over the Jimmies, while the violence of the confrontation costs Kelson his life. The scene shows a power shift, not a settled future for the survivors.
