Why Arthur rejects the Joker
At the start of the film, Arthur is in Arkham awaiting trial for his crimes as Joker, as the official film listing and the official movie site establish. The conflict is not simply whether he can escape punishment. It is whether anyone—including Arthur—will see a vulnerable person behind the public identity that has taken over his life.
The Joker persona has become a performance that other people want from him. As the trial and the attention around him make the character central, Arthur is increasingly treated as a symbol rather than as Arthur. His eventual confession and rejection of that identity are an attempt to speak as himself, not a triumphant decision to become a better or more powerful Joker.
Why Lee leaves Arthur
Lee is drawn to the Joker image and the performance surrounding it, not simply to the exposed, vulnerable Arthur. When he repudiates that identity, he is no longer the figure she wants him to be. Her departure makes the cost of his choice immediate: he loses the relationship as well as the audience and protection the persona seemed to offer. TIME’s ending analysis also reads Lee as captivated by the Joker image.

Who kills Arthur in Joker: Folie à Deux?
A fellow inmate at Arkham fatally stabs Arthur after Arthur has rejected the Joker persona. The killing is not presented as a victory or transformation for Arthur. It closes his personal story at the moment he has insisted on being seen as himself.
Is the final inmate the “real” Joker?
The ending strongly suggests that the inmate may take up the Joker identity: he tells a joke and cuts his own mouth, evoking the imagery associated with the character. But the film does not give him a full backstory or confirm what he will do next. It is more precise to call him a possible successor to the symbol than to say the movie definitively establishes him as the Joker in a wider continuity.

That distinction is the ending’s central idea. Arthur dies as Arthur; the Joker can persist as an identity other people imitate, build myths around, or demand from someone else. A DC editorial by Jules Chin Greene interprets the ending in those terms, saying, “Ultimately, as the film reveals, this is why Arthur Fleck isn’t really the Joker to begin with, a point hammered home by the film’s arresting ending.” The piece is explicitly an editorial interpretation, not a statement on behalf of DC or Warner Bros. TIME likewise describes the final twist as shifting the story’s Joker identity away from Arthur. These readings support the symbolic implication, not a definitive account of the inmate’s future.

Not in any ordinary sense. Arthur does not reconcile himself to the Joker or receive a hopeful resolution. He chooses to identify as Arthur, and the film ends with his abandonment and death. What survives is the disturbing possibility that Gotham’s appetite for the Joker will outlast the man who refused to be him. Director Todd Phillips’s and Joaquin Phoenix’s reported interpretations of Arthur’s final state and fate are discussed by Variety Australia; they are perspectives on the ending, rather than a reason to treat the final inmate’s future as settled fact.
