Meredith Grey and Derek Shepherd’s relationship begins with a one-night encounter before either knows the other’s name, then collides with Derek’s existing marriage. They separate, rebuild their relationship, make vows on a Post-it, raise three children, and face career and family upheaval before Derek dies in Season 11. This timeline follows the show’s season and episode milestones; it does not assign an exact in-story duration, which the available chronology does not establish reliably.
Before Seattle Grace: they meet in a bar
Before Meredith starts her surgical internship, she meets Derek in a bar. They spend the night together without first exchanging names. ABC’s Meredith Grey character guide describes the encounter as the beginning of a relationship that eventually leads to marriage; Derek later recalls meeting “this girl in a bar” in ABC’s recap of his final episode.
Season 1: the stranger becomes her attending
At Seattle Grace, Meredith discovers that her bar-date is Derek Shepherd, a surgeon and her attending. Their attraction now sits alongside a professional hierarchy that complicates their private relationship. ABC’s Meredith and Derek highlights recalls their first scene before they know each other’s names and Derek asking Meredith out despite the boss-and-intern complication.
Season 1 finale and Season 2: Addison changes everything
Just as Meredith and Derek appear to be moving toward a committed relationship, Addison Montgomery arrives and reveals that she is Derek’s wife. The revelation exposes the central problem: Derek had not ended his marriage when Meredith believed their relationship might become a partnership. ABC’s highlights and Meredith’s character guide both identify Addison’s entrance as the rupture in their early romance.

Seasons 2–4: separation, reunion, and a slower start
Derek confronts the end of his marriage to Addison, and he and Meredith eventually try again. Their reunion is not an instant return to how things were: ABC points to Season 3’s “Staring at the Sun” (Season 3, Episode 8) as a renewed-relationship milestone, with the two agreeing to take it slowly.
Over the following seasons, the relationship is tested by Meredith’s near-drowning, grief, her complicated history with her mother Ellis, and her fear of committing to a stable future. Derek supports Meredith through the aftermath of the drowning, but the broader arc is one of repeatedly negotiating trust and timing rather than solving everything with a single reconciliation.

Season 5: proposal, then Post-it vows
Derek proposes in an elevator. Meredith’s planned wedding ceremony is redirected when she gives it to Izzie Stevens and Alex Karev. Instead, Meredith and Derek make their own vows on a Post-it. Netflix’s listing for Season 5, Episode 22, “What a Difference a Day Makes”, identifies the Post-it wedding; ABC’s Meredith guide clarifies that the proposal came first. The Post-it is their marriage milestone, even though they do not have the public ceremony originally planned.
Season 6: surviving the hospital shooting
The hospital shooting puts Derek in immediate danger and makes Meredith’s family and relationship central to the crisis. ABC includes Derek being shot among the couple’s defining moments. It is a test of their life together, not a separation: they remain married.

Season 7: they begin building a family
Meredith and Derek pursue adoption, and Zola becomes their daughter after the process brings uncertainty and separation. ABC’s highlights single out Derek’s wish to adopt Zola as a turning point for the couple. Their relationship is now shaped not only by their commitment to each other but by the family they are trying to create.
Season 8: the plane crash and a shared future
The plane crash kills Lexie Grey and leaves Derek with a badly injured hand; Meredith survives. The trauma changes what the future looks like for both of them without ending their marriage. ABC’s highlights also recall the house associated with Meredith’s hopes for their family.

Seasons 9–10: children and two demanding careers
Meredith and Derek raise Zola and welcome their son Bailey, named for Miranda Bailey. Meredith goes into labor during a blackout, and Bailey’s actions help save her. ABC’s character guide confirms the family’s expansion, while the retrospective notes Derek’s excitement about another child. The couple later learns they are expecting a third baby.
At the same time, both doctors are building demanding careers. Derek’s work draws national attention as Meredith establishes herself as a surgeon and mother. The strain is not just a single choice between a career and a marriage; it grows out of two ambitious careers and the practical responsibilities of raising young children. Season 11: Washington, return, and Derek’s death
The Washington conflict
Derek accepts a White House-related job in Washington, and his decision and the distance strain the marriage. In ABC’s character guide, the couple’s argument ends with Meredith telling him to take the job. ABC’s highlights later describe Derek’s return to Meredith because his heart was not in Washington. The established sequence is conflict, time apart, and renewed commitment; an exact duration is not reliably established.
“How to Save a Life”
In Season 11’s “How to Save a Life,” Derek helps people at a road crash, is hit by another vehicle, and is taken to a hospital where he does not receive timely, appropriate care. Meredith arrives, recognizes what the records mean, and is present when life support is withdrawn. ABC says the episode originally aired April 23, 2015, in its episode recap. Derek’s death ends their shared relationship timeline.
Meredith’s life after Derek
Meredith leaves Seattle with her children, grieves, and later returns. She gives birth to Ellis, Derek’s third child, after his death. ABC’s “She’s Leaving Home” recap follows Meredith’s grief, pregnancy, and the baby’s birth. Later appearances or references to Derek belong to the aftermath and remembrance of their relationship, not a resumed partnership.

How much time passes between their first meeting and Derek’s death?
The season and episode order establishes the milestones, but it does not support a precise elapsed-time answer. Grey’s Anatomy’s fictional chronology is compressed and sometimes inconsistent, so real-world air dates cannot be used to calculate how many years pass for Meredith and Derek. The safest answer is that their story runs from before Meredith’s internship through Season 11, rather than a specific number of months or years.
