Iron Lung turns a compact horror-game premise into a 127-minute feature that prizes confinement and unease over momentum. Mark Fischbach’s commitment to that claustrophobic approach gives the film a distinctive atmosphere, but its deliberate monotony can make the descent feel longer than it needs to.
What is Iron Lung about?
After the Quiet Rapture wipes out known stars and habitable planets, a convict is sent in a small submarine to explore an ocean of blood on a desolate moon. The official synopsis frames the mission as a search for resources, while the film’s horror grows from what the confined explorer cannot see or safely understand.
Mark Fischbach—known to audiences as Markiplier—is credited as the film’s writer, director, editor and executive producer. It is based on Iron Lung, the horror game by David Szymanski. The official site lists the feature at 2 hours 7 minutes and rates it R.
Is Iron Lung slow-paced?
Yes, by design. Fischbach has said that his first rule was to keep the story inside the submarine. He describes monotony, loneliness and boredom as part of the experience he wanted to preserve, not problems to edit away. As he put it in a February 2026 interview with the YouTube Official Blog, “The point is the monotony, the loneliness, the boredom.”

That intention gives the film a clear aesthetic: the submarine is a restrictive box, and the plot depends partly on the limits of its occupant’s perspective. Fischbach has also argued that “the plot is what you aren’t being told.” The approach can make an ordinary stretch of waiting feel ominous, but a sustained atmosphere is not the same as sustained tension. At 127 minutes, repeated confinement and mission routines may feel immersive to viewers who enjoy slow-burn horror and overextended to those waiting for the story to change shape.
How does the film compare with the original game?
Szymanski’s original is a short game, released on Steam on March 9, 2022. Steam describes the player piloting a tiny submarine through a blood ocean on an alien moon. The feature starts from that compact setup and expands it into a much longer narrative, while retaining the submarine as its central viewpoint.

The key adaptation question is whether more story makes the game’s mystery richer without explaining away its unease. The film’s extended runtime allows it to linger on isolation and danger, but the same restraint that protects the game’s mood can leave the feature feeling repetitive. The sources establish the shared premise and Fischbach’s stated approach; they do not establish a competing film adaptation for comparison.
Does the horror work—and is it cosmic horror?
The film’s strongest horror tool is scale made intimate: an ocean of blood and an unknown moon are encountered through a cramped vessel with limited visibility. That combination supports a cosmic-horror reading, since the threat is vast, alien and only partly knowable. But the label alone does not guarantee suspense. The film’s effect depends on whether the viewer finds dread in restricted information and long stretches of waiting.

Reception reflects that divide. When accessed on October 4, 2026, Rotten Tomatoes listed a 59% Tomatometer from 49 critic reviews and an 86% Popcornmeter from more than 2,500 verified ratings. Its critics’ consensus praised the atmosphere while warning that the film could be stifling; the displayed reviews ranged from positive slow-burn responses to complaints about length and repetition. Those mutable scores describe the site at that access time, not a permanent verdict.
